• For Oral Paper Presenters
• For Oral Paper Presenters with Formal Discussion
• For Rapid Fire Medical Student Papers
• For Poster Presenters
• For Workshop Presenters

2. Financial Disclosure Form - ALL POSTER/PAPER/WORKSHOP PRESENTERS

• Only required at this time if your original disclosure (submitted with abstract) has changed or if you have not yet completed since September/October 2024.
